Click here for the new athletic physical packet for the 2011-2012 school year. Athletes may fill out the highlighted areas and then print it off to take to their physician. Hard copies of the packet will be available by the first of May at the Athletic Facility. The physical exam must be done after May 1st, 2011 to be considered current for the 2011-2012 school year.
